Our Pre & Post-acute rehabilitation programs are intended to help patients transition from the hospital back to their pre-hospitalization health. We also provide respite care when needed for caregivers.
Rehab Services
Helping individuals regain functionality following an illness, accident, hospitalization or surgery is the focus of the goal-oriented rehabilitation services we provide. Throughout the Diocesan Health Facilities group, we offer a mix of short-term and extended care rehabilitation programs to help people get back to their functional best.
Our dedicated team of rehab professionals will work with you or your loved one each step of the way along the road to recovery. Whether the goal is to improve mobility, address the activities of daily living or improve communication skills, our physical, occupational and speech/language therapists are ready to help. We also specialize in therapeutic rehabilitation care for those with respiratory disorders or pulmonary disease.
For those returning home following rehabilitation care, we work to make the process as streamlined and seamless as possible to ensure a successful transition. We do this by conducting a home visit to assess and prepare the individual for independent living. Making connections to the appropriate community agencies provides a continuum of care.

We Do Rehab Right!
Respite Care
Our Respite Care program provides planned short-term accommodations for loved ones whose families are their caregivers. Respite has been shown to help sustain family caregiver’s health and well-being. Respite is often used when:
- The family caregiver needs to travel.
- The family caregiver needs a break from their daily routine.
- As a trial period to decide if a senior community is most beneficial for their loved one.
