We are a leader in independent living, memory care, skilled nursing, short-term rehab, and respite care programs. With differing levels of care, we will help pair the appropriate type of care for you or your loved one.
Skilled Nursing Care
We offer short- and long-term nursing care for adults of all ages – from those recovering from surgery or illness with a goal of returning home to the frail elderly, many who make us their home. Regardless of each individual’s medical profile, the goal is always the same: to restore and maintain the highest level of physical function and personal independence.
We do this by creating customized treatment plans to address each resident’s individual needs. Our skilled nursing team is complemented by rehabilitation therapists, dieticians and social workers to deliver the optimum level of care, no matter how complex the plan. Innovative pain management, palliative care, and access to hospice programs ensure the best quality of life possible, even in its final moments.
Ongoing pastoral care is integral to our approach, with access to on-site daily Mass and a broad range of engaging activities to stimulate personal reflection and social interaction. We are proud to meet the needs of the local community by offering bilingual staff members, religious services spoken in Portuguese at some locations, and activities reflecting the culture.

Rehabilitation Services
Helping individuals regain functionality following an illness, accident, hospitalization or surgery is the focus of the goal-oriented rehabilitation services we provide. Throughout the Diocesan Health Facilities group, we offer a mix of short-term and extended care rehabilitation programs to help people get back to their functional best.
Our dedicated team of rehab professionals will work with you or your loved one each step of the way along the road to recovery. Whether the goal is to improve mobility, address the activities of daily living or improve communication skills, our physical, occupational and speech/language therapists are ready to help. We also specialize in therapeutic rehabilitation care for those with respiratory disorders or pulmonary disease.
For those returning home following rehabilitation care, we work to make the process as streamlined and seamless as possible to ensure a successful transition. We do this by conducting a home visit to assess and prepare the individual for independent living. Making connections to the appropriate community agencies provides a continuum of care.

Respite Care Program
Our Respite Care program provides planned short-term accommodations for loved ones whose families are their caregivers. Respite has been shown to help sustain family caregiver’s health and well-being. Respite is often used when:
- The family caregiver needs to travel.
- The family caregiver needs a break from their daily routine.
- As a trial period to decide if a senior community is most beneficial for their loved one.

Alzheimer’s / Memory Care
Catholic Memorial Home and Sacred Heart Home are among the few facilities in Southeastern Massachusetts with dedicated Alzheimer’s units. We understand the unique challenges facing stricken individuals and their families; respect for each resident’s individuality, dignity, and comfort define the specialized care we provide within these facilities.
Specially trained staff, therapeutic living environments, and ongoing pastoral care combine to promote the best quality of life for our residents with Alzheimer’s disease and other forms of memory loss and dementia. Within this safe and secure setting we nurture by emphasizing individual strengths, providing programming to help preserve cognition skills, encouraging positive activities, and fostering emotional and spiritual well-being. In turn, this engenders a sense of value and purpose for the residents we serve and provides comfort to their families.
Our Memory Care is designed to help those with memory impairments to stay active and engaged, supported by a caring staff, and specialized programs designed just for them. Our Memory Care is led by specially trained care staff who provide the programming, services, and support to help residents feel secure, comfortable, and successful every day.

Pain Management
The Diocesan Health Facilities’ pain management program was instituted to deliver optimum relief and elimination of acute or chronic pain for all residents, regardless of age, illness, or mental health status. A comprehensive pain assessment begins upon the resident’s admission and continues throughout the duration of their stay. Medically advanced assessment tools allow us to provide comfort and care to cognitively impaired residents.
Through specialized training and continuing education, the pain management staff cares for the most complex needs of our residents. All nurses are educated in the use of intravenous drug therapies to administer pain-relieving medications as prescribed by physicians. Our holistic approach to pain relief encompasses non-drug techniques and proven therapies, including therapeutic touch, relaxation, music, meditation, nerve stimulation, and physical therapy.
Pain Management
Palliative Care & Hospice
Our palliative care program, offered at each of the Diocesan Health Facilities skilled nursing centers, provides terminally ill residents with specialized treatment, including appropriate and aggressive pain management, and compassionate support. End-of-life care is coordinated by a palliative care team, which may comprise the resident’s physician, nurse, and pastoral care. Special Care Rooms at each of our facilities allow residents and family members to experience a private room with accommodations and conveniences that provide medical services in a serene setting.
The resident and their family members receive guidance concerning important decisions such as the need for hospitalization. Our affiliations with several hospice agencies facilitate coordination, allowing us to support residents and family members by maximizing time together, managing symptoms, and assuring peace and dignity at the end of life. Upon the resident’s death, the pastoral care director or social service staff provides and coordinates bereavement care for the family.
